As per the information provided by EPFO in the video the Validity period is of one year from the date of submission. 

For getting the authentication done one will require the following details:

1. Aadhaar Number

2. PPO Number

3. Bank Details

4. Mobile Number

However, no document is required after the submission of Digital Life Certificate.
