Educational Qualification:
The candidate must have done B.A.M.S. or B.H.M.S. from a recognized University with three years of experience after the qualification experience. 

Age limit:
The candidate should not be exceed to 65 years as on 1st July, 2019. 

Remuneration:
Candidate will be paid with Rs. 600 per visit (all inclusive) through bank after deduction of applicable Tax. Candidate should have Income Tax Pan card at the time of joining. 

Procedure to apply:
The interested candidates submit their application form along with copy of necessary documentary proof in support of Educational Qualification, Experience, Registration Certificate with Medical Council of India, SSLC / + 2 Marks Sheet in support of Age Proof, to reach SR.
