On Friday, October 4, at 7:54 PM, the gate man on duty at Gate No. 333-C noticed something unusual while allowing the train to pass; he observed sparks suddenly flying from the train's wheels. Recognising the potential danger, he immediately informed the station superintendent, who promptly notified the locomotive pilot.

Despite the loco pilot's application of the emergency brake, the rod had already become stuck in the engine.

Authorities at the Jakhaura police station in Lalitpur have registered a case against unidentified individuals and launched an investigation into the matter under BNS Section 327(1) and Railway Act Section 150.
